PHYSICIAN LICENSURE EXAM REQUIREMENTS

1. Original copy of BIRTH CERTIFICATE in NSO Security Paper and Birth Certificate in LCR Form-1A.

2. Original and Photocopy OF TRANSCRIPT OF RECORDS (Special Order No., Date of Graduation, with Scanned Picture and additional remarks: "For Board Exam Purposes" must be indicated in the TOR)

3. Four (4) passport size colored pictures with NAME TAG and in white background. Guys should wear polo.

4. Current Community Tax Certificate (CEDULA)

5. Certificate of Internship
